San Francisco 49ers quarterback Alex Smith won't play Monday against the Chicago Bears as he continues to recover from a concussion.

Smith was hurt last Sunday against St. Louis when he took a hit to the neck area and had been listed as questionable on the team's injury report after being a limited participant in practice this week.

The 49ers updated Smith's status Monday and said he was not medically cleared to play.

"Alex Smith has been declared out for this evening's game after being evaluated by team medical director Dr. Dan Garza," the team said in a statement.

Colin Kaepernick will start Monday against the Bears in a what will be a battle of backups, as Jason Campbell will start at quarterback for Chicago while Jay Cutler also recovers from a concussion sustained last week.
