Baldwin was headed east toward Longview at about 9:40 p.m. in a 2011 Chevy Aveo when he crossed the center line and struck the westbound van, the memo states.

Driver Todd D. Wilson, 55, was sent to Portland's Legacy Emanuel Medical Center.

His passenger, Mason D. Wilson, 24, was lifeflighted to the same hospital.

His other passenger, Nicole S. Wilson, 55, was taken to PeaceHealth St. John in Longview.

Negligent driving is listed as the cause of the accident, but it is not known if drugs or alcohol were involved.
